I've come across a lot of people referring to her as E. E. Nesbit, and I've never actually seen a picture of a book jacket that has a second E. Most of the old book jackets just seem to refer to E. Nesbit, and her Wikipedia doesn't have any information on a middle name for her.
Was 'E. E. Nesbit' ever a name she was known by, or is it one of those urban myths? (I did wonder if maybe people are thinking of e. e. cummings?)

Never. Just E. Nesbit. Perhaps you are right in the confusion with E. E. Cummings. There is also A. A. Milne which might be confusing people.
