My grandmother has a family bible from the 1860s, in which is recorded many family relevant dates (birth dates, marriage dates, death dates) most of which were added pre-WW2. She wants to add some more relevant stuff in there such as recent births, marriages, and deaths.
Is there any danger in harming the book?
Does a special kind of ink need to be used?
Is it better to put a modern page of modern paper in the book and write on that instead?

You’ll get different opinions, but I say go for it. This is your family’s book and there are plenty bibles around, so it’s not like you’re writing in the only one in existence. Look at pens with archival ink (micron are particularly good). If you’re going to insert a modern page in, make sure you use archival/acid free paper.
You’re not selling it. It’s more valuable to your family than anyone else. I’d add those names and dates.
