Slightly over 20 years being published in 2000, but House of Leaves gets my vote.

What do you mean by difficult? Are we talking about prose or vocabulary or plot

What do you define as difficult prose? Flowery/purple prose? Lengthy sentences with lots of subordinates?
I’d say a good contender is Solenoid by Mircea Cartarescu (I’ve been talking about this book more and more in the last few days)

The Covenant Of Water, Abraham Verghese. Language is hard and makes me lose the story

Gideon the Ninth by Tasmyn Muir. The entire series is well known to be confusing due to the high diction, crazy prose, and purposefully confusing plot.
This Is How You Lose the Time War by Max Gladstone and Amal El-Mohtar is well known for its flowery and “purple” prose. I personally loved it, it’s one of my favorite books, but I can see why people would hate it.

Ducks, Newburyport
by Lucy Ellmann. 1000+ pages, list-like stream-of-consciousness uh, something?? I started it. It didn’t go into my DNF, it went into Try Again Another Time. So yeah – really hard to read but seems like it may be worth it.
