So this is almost assuredly not _exactly_ what you’re looking for because it’s a madcap action thriller — yet Richard Condon’s _The Whisper of the Axe_ totally satisfies this prompt and it’s also an unjustly forgotten book, coming out in 1974 if I remember correctly.
Condon is most famous for his late 50s political conspiracy masterpiece _The Manchurian Candidate_, yet he published ~26 novels in list lifetime, which are always at least interesting and different and usually intense / innovative. All pretty much forgotten besides a small handful. To me when at his best he’s like a non-SF Philip K Dick, and I enjoy reading one of his books when I want to “go sane.” And _The Whisper of the Axe_ is one of my very favorite books of his
